Healthy Pathways
Phase 3  | Current

Live Well Geary County’s successful distribution of Phase 1 and Phase 2 Pathways funding led to the award of Phase 3 Pathways funding.

​

Phase 3 funding involves six Pathways: Community and Social Context, Neighborhood and Physical Environment, Healthcare, Education, and Economic Stability. Some possible projects for Phase 3 Pathways funding are: renovations to a site for accessibility, community kitchen, expanding community gardens, assisting in Community Health Assessment Needs survey, vaping/JUUL prevention and education, and healthy food access to educational sites. 

Library

Community & Social Context

Focus on work at the community level to improve social integration, support systems, community engagement, and reduce stress and discrimination.

Placemaking/Library

Activity Summary: Research from the University of Oxford has revealed that the more often people eat with others, the more likely they are to feel happy and satisfied with their lives. The researchers looked at the link between social eating and an individual’s happiness, the number of friends they have, their connection to their community, and overall satisfaction with life. The results suggest that communal eating increases social bonding and feelings of well-being, and enhances one’s sense of contentedness and embedding within the community. The Administration for Community Living’s Congregate Meals Program for adults 60+ has shown communal meals can improve health for that population, provide access to food and increase social activity.

Activities include identifying a structure plans were developed with input from the community for renovations to the Dorothy Bramlage Public Library. Plans include an outdoor activity area for kids. There was also a need identified for accessibility to bus stops at the Library. .

Fresh Organic Vegetables

Economic Stability

Represents an individual’s ability to access resources such as food, housing or healthcare, as well
as creating a healthy business culture to help an employer attract and retain talent, reduce absenteeism and turnover, and improve productivity.

Healthier Food Retail/Farmers Market

Activity Summary: Farmers markets provide communities with sources of locally grown, fresh produce during the growing season while also providing local producers with additional retail opportunities to sell their produce. The success of a local farmers market depends on the availability of local growers to provide fresh produce, a commitment from local community members to buy produce at the market, and access to a space for the farmers market to operate. Farmers markets that accept EBT and other food benefits can create opportunities for low-income community members to obtain fresh, local and healthy food.

Action: Geary Community Farmers Market received Phase 2 Pathways implementation funds to provide additional incentives for food benefit users and seniors, marketing and promotion, and kid’s nutrition activities. For sustainability, work has started to move toward combining the Thursday night GCFM and the Saturday Junction City Main Street Markets to operate as the Junction City Markets. Last season the markets were in different locations but shared a market manager and staff. For the 2025 season, the markets will be in the same location and operate with one manager and a staff of market coordinators. A Farmers Market online management platform has been implemented to make market operations more efficient. 

Gardening

Neighborhood & Physical Environment

Focus on building connections between where a person lives (e.g., housing, neighborhood, and
environment) and his or her health and well-being.

Gardens and Community Farm

Activity Summary: This package was selected to expand and sustain the 12th Street Community Garden. Junction City has a successful community garden located at the city-owned 12th St. Community Center. The gardens include individual plots and raised beds that were purchased through donations and grants. There is a plot in the garden reserved for homeschoolers to learn about gardening and nutrition education. Extra produce grown at the garden is donated to the Live Well Geary County-operated Healthy Together Senior Community Meals to distribute to patrons and for use in Healthy meal preparation. Produce is also given to the Geary County Food Pantry for their Fresh Food Friday’s distribution.

Activities could include the purchase of additional garden beds, looking at ways to expand the garden to include local producers and farmers market vendors, and continuing to be able to pay a maintenance person during garden season. The garden is located in a lower socio-economic area close to an income-based senior living facility and is open to anyone in the community wishing to plant a garden.

Making Vending Machine Purchase

Education

Creating connections between education and health and well-being by promoting healthy lifestyles,
educational attainment, language and literacy skills, and early childhood education and development. 

Healthier Higher Education Food Environment/

Activity Summary: Work has been done with several organizations in the community to encourage healthier behaviors. Discussions with Cloud County Community College Geary campus have been about the needs of their students for healthy vending machines, nutrition education, and the growing need for expanding their student food pantry. Cloud County Community College serves the very diverse population of Geary County and the surrounding area. Students are from high school age to non-traditional adults. The campus offers GED certification to the adult population.

Activities could include improving the pantry and implement HER standards. Adding some opportunities for cold storage for student lunches in the breakroom and providing healthy meal prep classes with recipes and healthy food bags.

Healthcare

Focus on connecting access to healthcare and people understanding how health services impacts
their own health.

Health Related Social Needs

Activity Summary: Results from the 2024 Community Health Needs Assessment conducted by Stormont Vail Hospital and community health partners Geary Community Health Department, Konza Prairie Community Health Center, Live Well Geary County, Pawnee Mental Health, and USD 475 showed that the top health priorities for residents were adult obesity, access to community resources, transportation, mental health, and youth character development.

 

Activities could include hosting workgroups to engage the community in developing a Community Health Improvement Plan. The work is intended to help implement intervention strategies identified in the plan.
